Advanced Care for those facing chronic or terminal illness

VNA Health Group and Parker Health Group have recently partnered to launch the Parker Advanced Care Institute at VNAHG, Inc. (PACI). ).  Each organization with over 100 years of experience in caring for the aging population have joined forces in an effort to bring additional resources and programs to more seniors in their homes.  Core services of PACI include Visiting Physician Services and Palliative Care which are dedicated to helping patients with late-stage chronic illnesses stay comfortably at home. The goal of PACI is to educate clinicians, communities, patients and their families on best practices in chronic disease, palliative and hospice care. 

 

Parker Advanced Care Institute: Better Living, Less Hospitalizations

A key component of the Parker Advanced Care Institute is Visiting Physician Services (VPS), a home based primary care practice for older adult and homebound patients whose mission is to enable patients to stay at home as they age with an improved quality of life.  House calls provide patients and their families with comfort, safety and convenience. Another service offered through PACI is our Palliative Care Program. Palliative care is an individualized approach to addressing physical, emotional, cultural and spiritual needs of patients and families facing a serious or life-limiting illness.  The primary goals of a palliative care program are improving quality of life through management of symptoms, including pain, that are related to the illness.  We help patients and families navigate treatment options for those with a serious condition and provide care, support and compassion when it’s needed most.
