What is the Program?

Monmouth ACTS Navigation System provides Monmouth County residents with a comprehensive and accessible way to connect with resources in their communities. The Service Coordinators manage and provide access to necessary supportive services in the community. They ensure that residents are accessing the services they need in a timely manner and they understand how to use those services.  The Service Coordinator evaluates the needs of the resident, makes recommendations for new services, and/or offers suggestions to improve or better coordinate existing services.  The Service Coordinator explains available options, works with the resident to develop a plan, identifies any barriers and address those barriers.

    CFHI

    The Children and Family Health Institute (CFHI) is a charitable, nonprofit program of VNA Health Group. Contributions from individuals, foundations and corporations provide funding to help ensure the sustainability and growth of the CFHI. A variety of giving opportunities are available including permanent naming opportunities, endowments, special events, planned gifts as well as grants to support innovation and research.
