School-Based Nurse Practitioner Program

Nurse Practitioners provide primary health care services at the school for children in Pre-K to high school. Services may include diagnosis and treatment of common childhood health issues, medication prescriptions, health screenings, and management of chronic diseases such as asthma and diabetes. Nurse Practitioners also provide health education and referrals to community resources to meet the students’ needs.

    Locations

    The following school districts currently have Nurse Practitioners: Asbury Park, Keansburg, Freehold Borough, Red Bank Borough, Keyport (K-8), Eatontown and Toms River (Walnut Street School & South Toms River).

    Eligibility

    All students attending the schools in the districts above are eligible

    None

    Cost

    No cost to the student; Medicaid is billed for health visits, if applicable.

    The Children and Family Health Institute (CFHI) is a charitable, nonprofit program of VNA Health Group.
