National Health Center Week

National Health Center Week (August 9-15) is an annual celebration that raises awareness about the accomplishments of America’s health centers over the last 50 years. For this year’s National Health Center Week, health centers across the nation are “lighting the way for healthier communities today and in the future.”

VNA of Central Jersey Community Health Center (VNACJ CHC) is pleased to be a Federally Qualified Health Center (FQHC) that improves the health and well-being of individuals by providing high quality, cost-effective, comprehensive health care services.  As an FQHC, our mission is to build healthy communities & enhance well-being.

In honor of National Health Center Week, we are thrilled to share with you the strides we have made in home health, hospice care and visiting physician services.

What is a Health Center? How Does it Differ?

Health centers are community-based organizations that deliver comprehensive health care services to people in need. They tend to be located in areas where economic, geographic or cultural barriers limit access to affordable, quality healthcare services. These health centers also tend to focus on the most vulnerable populations such as residents of public housing, those experiencing homelessness and veterans.

Visiting Nurse Association of Central Jersey Community Health Center, Inc. (VNACJ CHC), a Federally Qualified Health Center (FQHC), has provided comprehensive primary and preventive health care for infants, children, and adults of Monmouth County for more than 27 years. The VNACJ Health Centers are conveniently located in Asbury Park, Freehold, Keyport and Red Bank. In 2019, our staff provided more than 44,500 visits to more than 14,200 patients.

VNACJ CHC is a Federally Qualified Health Center

VNCJ CHC is proud to be a Federally Qualified Health Center. According to the Health Resources and Services Administration (HRSA), FQHCs:

● Offer a sliding fee scale

● Serve an underserved population

● Qualify for Medicare and Medicaid benefits

● Provide comprehensive healthcare services

● Have an ongoing quality assurance program

● Have a governing board of directors

With the ability to refer families to quality CFHI services at young ages, as well as Home Care and Hospice services in later stages of life, VNACJ is here to be a part of your life from birth to death.  No matter what the need is, VNACJ is here to help and serve you.
