Improving Your Quality of Life with In-Home Care

 

Receiving nursing care and rehabilitation at home following a stroke can help you begin your recovery and restore losses to speech and movement, and without the risk of further injury by traveling to a rehab facility or hospital.

 

Our multidisciplinary team includes nurses, physical therapists, occupational therapists, speech-language pathologists and other clinicians who work with you, your doctor and caregiver to regain your independence and quality of life. If you have Medicare or Medicaid, a series of nursing and rehabilitation visits are often covered.

Additional care options include Visiting Physician Services, which provides primary care along with lab work and medication management to your home with care from our team of house call doctors, physician assistants, and nurse practitioners. Covered by Medicare Part B, Visiting Physician Services is a long-term solution to helping you manage your health and remain independent.


If you would like to supplement the care provided to you under Medicare or Medicaid or would like personal assistance from a Certified Home Health Aide, our Private Care services can help.
