Our Joint Venture Partners
Trusted Home Health, Hospice and Visiting Physician Care Services in NJ Healthier People Make Healthier Communities. Without strong partnerships with New Jersey’s leading health systems, Visiting Nurse Association Health Group could not accomplish its mission of providing health care to patients in New Jersey. Our unique partnership model with Robert Wood Johnson University Hospital, Englewood Hospital and Medical Center, and RWJBarnabas Health leverages a wide network of clinical expertise to help you achieve your best level of health.
The goal of our home care agency is to treat an illness or injury while helping patients regain their independence and become as self-sufficient as possible. To do this, we collaborate with the patient’s care team and family to customize a care plan and coordinate a wide range of therapies from doctors, registered nurses, therapists, and other health care professionals. Medical social workers can assist in coordinating resources and support, as well as long-term care planning.

We offer a range of rehabilitation therapy disciplines delivered by highly-skilled physical therapists, occupational therapists and speech therapists to help you recover and regain your independence. Joint-replacement therapy at home is becoming the preferred method for patients who are undergoing hip or knee replacement. Our joint-replacement rehab team has a track record of helping patients bounce back faster. For wound care, we have partnered with Parable Health to link our certified wound care nurses with the highly-effective and HIPAA-compliant wound care app that maximizes healing and health outcomes. Our skilled multidisciplinary team helps patients who need ostomy, catheter or IV-therapy care in the comfort and privacy of your home to help you achieve a dignified, independent life. Our patients also receive a thorough medication review from their nurse and our in-house doctor of pharmacy (PharmD) to identify possible drug-interactions and to assist you and your caregivers with a proper medication regimen.
