Healthy Families

The Healthy Families Program supports pregnant women or parents/guardians through home visits where a specially trained Family Support Worker brings education and resources to parents and children. Some of these resources include pregnancy, parenting, child development, and nutrition education; other community services include linkages to job training, schooling, child care, and transportation. Home visits last for up to three years.

    Locations

    Monmouth, Middlesex, and Essex Counties

    Eligibility

    Enrollment available to pregnant women or new moms or dads up to three months after the birth of the baby.

    Insurance/income Requirements

    If you or your family qualifies for Medicaid, TANF, or WIC, you are eligible for this program. For families receiving TANF, children up to age 12 months of age may be enrolled.

    The Children and Family Health Institute (CFHI) is a charitable, nonprofit program of VNA Health Group.
