Telehealth Monitors your Daily Health

Telehealth is an innovational program of in home monitoring that offers daily nursing support in addition to traditional in-home visits. The VNA has the ability to monitor a patient’s daily weight, blood pressure, heart rate and oxygen levels remotely using Telehealth monitors delivered to their home shortly after admission. Readings are transmitted directly to our telehealth nurses for evaluation and intervention as needed. The goal of this program is to help our patients learn to better manage their health while recovering safely in the comfort of their own home.

Home Care

For wound care, we have partnered with Parable Health to link our certified wound care nurses with the highly-effective and HIPAA-compliant wound care app that maximizes healing and health outcomes. Our skilled multidisciplinary team helps patients who need ostomy, catheter or IV-therapy care in the comfort and privacy of your home to help you achieve a dignified, independent life. Our patients also receive a thorough medication review from their nurse and our in-house doctor of pharmacy (PharmD) to identify possible drug-interactions and to assist you and your caregivers with a proper medication regimen.
